Job summary

Suvida Healthcare in Tucson, AZ is seeking a compassionate Neighborhood Center Physician to lead outpatient care within a value-based model for Medicare-eligible seniors. You will diagnose, manage, and coordinate care with a multidisciplinary team to close gaps and improve quality of life.

The ideal candidate is an MD or DO, board-certified or eligible in Family or Internal Medicine, with 3+ years in geriatrics, a valid Arizona license, and strong leadership with English/Spanish communication.

Qualifications

• 3+ years minimum experience, preferably in geriatrics.
• Active, unrestricted medical license in Arizona.
• Board certification or eligibility in Family or Internal Medicine.
• Current DEA license.
• Bilingual/ bicultural English and Spanish preferred.

Responsibilities

• Cares for patients in outpatient medicine and focuses on continuity of care and disease prevention.
• Manages patients in a multidisciplinary value-based care model.
• Leads the care team within the Neighborhood Center.
• Collaborates with multidisciplinary care team to improve outcomes.
• Attends daily morning huddle and weekly care team huddle to discuss patients and address gaps.
• Oversees Advanced Practice Providers as needed within the Neighborhood Center.
• Reports to the Neighborhood Center Medical Director.

What You’ll Do

Position Summary

The Neighborhood Center Physician serves as a health-care professional who drives, supports, diagnose, and cares for a variety of health-related problems for Suvida Healthcare’s patients. The provider will manage these patients in a multidisciplinary value-based care model. The physician work assignments involve low-emerging risk complex patients to high complexity patients where the analysis of situations or data requires an in-depth evaluation of variable factors. The clinician will collaborate with a multidisciplinary care team and will serve as the leader of their respective care team within their Neighborhood Center. Essential responsibilities consist of but are not all inclusive:

Responsibilities

• Cares for Suvida Healthcare’s patients and focuses on outpatient medicine, continuity of care, health maintenance, improving health outcomes, and disease prevention
• Manages patients in a multidisciplinary value-based care model
• Serves as the leader for their respective care team within the Neighborhood Center
• Collaborates effectively with Suvida Healthcare’s multidisciplinary care team
• Focuses on providing 5-Star care and strives to provide a high level of customer service
• Keeps a medical history and medical records
• Refers the patient to specialists as needed
• Attends daily morning huddle with care team to discuss patients to be seen that day and works to close any potential care gaps
• Attends weekly huddle with multidisciplinary care team to review and manage high-risk patients within the care team
• Begins to influence department’s strategy
• Makes decisions on moderately complex to complex issues regarding technical approach for project components, and work is performed without direction
• Exercises considerable latitude in determining objectives and approaches to assignments
• Oversees Advanced Practice Providers as needed within their Neighborhood Center
• Reports to the Neighborhood Center Medical Director

What You’ll Bring

Knowledge, Skills, and Abilities

• 3+ years minimum experience, preferably in geriatrics
• Active, unrestricted medical license in Arizona
• Current on all required immunizations
• A working understanding and support of a managed care, full risk, value-based care model preferred
• Strong leadership, customer service, and spoken/written communication skills
• Must have the ability to demonstrate self-confidence and positive attitude toward self and others
• Excellent team player and communication skills required
• Proficiency in Microsoft office platforms including word, excel, and power point
• Bilingual/Bicultural (English and Spanish) Preferred

Education, Experience, Licensure, or Certification Requirements

• Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.)
• Board certification or eligibility in Family or Internal Medicine
• Current DEA license
