Job summary

Eventus WholeHealth seeks an Attending Physician to steward population health within its Outcomes-Centered Healthcare (OCH) model. You will lead care across client facilities and work closely with Advanced Practice Clinicians to deliver high-quality, team-based care with professionalism in relations.

The role may include Medical Directorship duties, providing clinical leadership and oversight in one or more facilities, while fostering interdisciplinary collaboration and proactive, whole-person

Qualifications

• MD or DO required; board certification in Internal Medicine, Family Medicine, or Geriatrics preferred.
• CMD certification a plus; required within 2 years if serving in a Medical Directorship capacity
• Active and unrestricted medical license in applicable state(s); DEA registration as required
• Minimum 2-3 years of clinical experience in post-acute, long-term care, or value-based care settings preferred
• Experience collaborating with interdisciplinary teams and facility leadership
• Familiarity with EMR systems (AthenaHealth preferred) and care coordination workflow

Responsibilities

• Provide clinical leadership and oversight within client facilities.
• Lead population health initiatives under Eventus OCH model and collaborate with APCs toward high-quality care.
• Partner with facility staff and leadership to ensure accountable, team-based patient care across assigned facilities.
• May serve in a designated Medical Directorship capacity and provide clinical leadership as assigned.

Position Summary

The Attending Physician serves primarily as a steward of population health through Eventus WholeHealth’s Outcomes-Centered Healthcare (OCH) model.
