Enjoy the flexibility of community-based work in Skilled Nursing and Assisted Living Facilities, leading a team of Advanced Practice Providers and collaborating with a multidisciplinary team including Psychiatry, Psychotherapy, Podiatry, and Wound Care.

Responsibilities include:

• Providing primary care according to site-specific protocols and standards for family, geriatric, or internal medicine in long-term care settings
• Reviewing and signing MARs, orders, labs, and managing prescriptions
• Allocating time for administrative duties during visits
• Participating in care plan, QA, and PI meetings
• Being available between 8AM-5PM via text or phone on scheduled days, with support from the ECare team
• Submitting documentation and billing records timely
• Maintaining open communication with facility staff and adhering to standards, policies, and legal regulations, including HIPAA and CPT guidelines

Qualifications:

• Licensed MD or DO
• Preferred 3+ years in Long Term Care/Geriatric Care
• AMDA Certification a plus
• Strong leadership and organizational skills
• Excellent communication skills
