Work Environment

Enjoy the flexibility of community-based work at Skilled Nursing and Assisted Living Facilities, leading a team of Advanced Practice Providers and collaborating with an interdisciplinary team including Psychiatry, Psychotherapy, Podiatry, and Wound Care where available.

Responsibilities:

• Provide primary care in accordance with site-specific protocols and standards for family, geriatric, and internal medicine in long-term care settings
• Review and sign MARs, orders, and labs within specified timeframes
• Manage medication prescriptions specific to your patients and facility policies
• Allocate time for administrative duties during visits
• Participate in care plan, QA, and PI meetings
• Be available for on-call duties between 8AM and 5PM, with support from our ECare team
• Ensure proper documentation and billing
• Maintain open communication with facility staff and adhere to all standards and regulations, including HIPAA and CPT guidelines

Qualifications:

• Licensed MD or DO
• Preferred 3+ years in Long Term Care/Geriatrics
• AMDA Certification a plus
• Leadership and organizational skills
• Excellent communication skills
