Job summary

Northeast Healthcare Recruitment seeks a physician to join our well-respected organization as a full-time or part-time primary care provider. This role focuses on outpatient visits, patient communication, and care transitions within a Patient Centered Care Model.

Qualifications

• Completed accredited physician residency in Family Medicine or Internal Medicine.
• Board Certified or Board Eligible; MA licensure and DEA eligibility.

Responsibilities

• Provide primary care visits to a panel of patients in outpatient settings.
• Coordinate safe transitions of care and refer to specialists.
• Promote disease prevention and practice evidence-based medicine.
• Communicate with patients and support the Patient Centered Care Model.
