Job summary

Northwest Human Services, Inc. is seeking a Physician/Primary Care Provider (MD/DO) for its West Salem Clinic in Salem, Oregon. The role supports a multi-disciplinary team delivering care to underserved populations in a Federally Qualified Health Center setting.

Clinic hours are Monday–Friday 7:00 a.m.–6:00 p.m. with Saturday coverage options. Salary ranges from $212,888 to $266,163, up to $319,384 DOE, plus a $21,000 sign-on and $5,000 referral bonus; provider compensation is salary-based.

Qualifications

  • MD/DO with Primary Care residency and board certification.
  • Licensed to practice in Oregon with prescriptive authority and DEA registration.
  • CPR certification (infant, child, adult) required.
  • Spanish language ability a plus; bilingual preferred.
  • Obstetrical care experience preferred.
  • Ability to work independently and flexible hours, including on call.

Responsibilities

  • Oversee care coordination including referrals, case management, and enabling services.
  • Supervise no more than two mid-level providers and proctor medical students as assigned.
  • Maintain accurate medical records and achieve >90% weekly documentation compliance.
  • Provide coverage for absent providers and participate in improvement teams.

Ph Physician / Primary Care Provider (MD/DO) – Family Medicine / Internal Medicine

Location: West Salem Clinic – Medical, 1233 Edgewater St. NW, Salem, OR 97304

Type: Outpatient Community Health Center / Federally Qualified Health Center (FQHC)

Hours: Monday – Friday 7:00 a.m. – 6:00 p.m.; Saturday 7:30 a.m. – 5:30 p.m.

Position Status: Flexible – Part‑Time or Full‑Time

Compensation: Salary ranges from $212,888 to $266,163, up to $319,384 (DOE). Sign‑on Bonus: $21,000 (internal candidates not eligible). Referral Bonus: $5,000 (expires at close of posting). Provider compensation works on a salary basis.

Position Overview

As part of the primary care treatment team, you will provide care coordination by working alongside our Community Health Workers, Medical Assistants, Nurses, Mental Health Case Managers, Counselors, Psychiatrists, Psych NPs, and other team members to support continuity of care for patients with complex needs.

Key Responsibilities
  • Oversee assigned patients’ care coordination, including specialty referrals, case management, and enabling services. Provide professional medical services including examinations, diagnosis, and treatment planning.
  • Assist nursing staff and physician assistants, serve as supervising physician to no more than two mid‑level providers, and proctor medical students as assigned.
  • Maintain accurate medical records in accordance with organizational policy, consistently meet documentation expectations, and maintain a weekly documentation report above 90% compliance.
  • Provide coverage for absent providers and serve on improvement teams.
  • Engage in organizational initiatives such as 340B usage, Patient Centered Medical Home certification, and Meaningful Use requirements.
  • Coordinate care with dental and behavioral health professionals in an integrated environment.
Qualifications
  • MD/DO completed Primary Care residency (Family Practice, Internal Medicine, or Pediatrics) and board certified.
  • Licensed to practice in Oregon with prescriptive authority and DEA registration.
  • Professional bondable and insurable.
  • Obstetrical care experience preferred.
  • Team‑oriented with strong interpersonal and communication skills.
  • Valid Oregon driver’s license and automobile insurance.
  • CPR certification in infant, child, and adult required.
  • Bilingual/bicultural strongly preferred; Spanish a plus.
  • Previous experience in community health preferred.
  • Ability to function independently and work flexible hours, including on call.
