Job summary

Fair Haven Community Health Care in New Haven, CT seeks a dedicated adult primary care physician to provide high-quality medical care in a busy outpatient setting. The role emphasizes patient-centered care, preventive services, and coordination with a team of clinicians.

The physician will document in EPIC, participate in performance improvement, and engage in community health initiatives. Connecticut license and BLS/CPR are required; bilingual (Spanish/English) is desirable.

Qualifications

• MD/DO from an accredited medical school.
• Completed internal medicine residency.
• BLS/CPR certification.
• Connecticut medical license.
• Spanish/English bilingual preferred.
• Experience in ambulatory/outpatient setting preferred.

Responsibilities

• Provides primary medical care.
• Develops a plan of care including medical history, examination, diagnosis and treatment; refers to hospitalization when necessary.
• Uses laboratory and other tests in diagnosis and treatment efficiently.
• Consults with clinicians, patients' families and staff regarding treatment and care when appropriate.
• Refers cases needing specialist services while maintaining primary care responsibility.
• Maintains accurate medical records in EPIC and performs charting and follow-up.
• Participates in Medical & General staff meetings and clinical teams.
• Participates in Performance Improvement program and continuing education.
• Performs other duties to deliver high-quality health care.

Job purpose

To provide high quality health care to the adult patients of the Fair Haven Community Health Center in the primary care setting consistent with FHCHC policies and mission.

Duties and responsibilities

• Provides primary medical care
• Develops a plan of care for each patient, including medical history, physical examination, diagnosis and appropriate treatment and/or referral, including hospitalization if necessary
• Uses all available resources in diagnosis and treatment, such as laboratory and others tests in an appropriate and cost efficient manner
• Confers with other clinicians, patient's families and other support staff concerning treatment and care of patients when appropriate
• Refers those cases which require specialist services, but maintains primary care responsibility to assure continuity of care
• Maintains accurate medical records within the EPIC electronic health record system and performs appropriate charting and follow up on patients
• Participates in Medical & General staff meetings and appropriate clinical teams
• Participates in the Health Center's Performance Improvement program, in other task groups as needed, and pursues continuing education opportunities to update medical knowledge and assure quality patient care
• Performs other necessary duties as required by the FHCHC to achieve the goal of providing high quality service and health care

Qualifications

• Graduation from an accredited medical school with a degree of Doctor of Medicine or Doctor of Osteopathy.
• Completion of an approved internal medicine residency program
• Bi-lingual in Spanish and English is desirable
• Experience in an ambulatory or outpatient setting preferred
• Ability to relate with warmth and effectiveness to patients and other staff
• Experience or specialty training in geriatric medicine is desirable
• License to practice medicine in State of Connecticut
• BLS/CPR certification required
