Job summary

Fair Haven Community Health Care in New Haven, CT is seeking a dedicated APRN to deliver high-quality primary care within our community health center.

You will develop comprehensive care plans, perform patient examinations, coordinate referrals, and maintain medical records while collaborating with a multidisciplinary team to ensure compassionate, accessible care for diverse populations.

Qualifications

  • APRN will provide high quality care in the primary care setting per FHCHC policies.
  • Develops a plan of care for each patient including history, exam, diagnosis, and treatment or referral.
  • Uses available resources (labs, tests) cost effectively in diagnosis and treatment.
  • Confers with clinicians, families, and staff about treatment and care as appropriate.
  • Maintains accurate medical records and performs charting and follow up.
  • Participates in staff meetings and clinical teams to ensure quality patient care.
  • Engages in Performance Improvement activities and continuing education to update knowledge.

Responsibilities

  • Provides primary medical care in line with specialty and FHCHC policies.
  • Develops care plans including medical history, physical exams, diagnoses and treatments.
  • Utilizes labs and tests in a cost-conscious manner.
  • Collaborates with clinicians, families, and staff on treatment decisions.
  • Maintains EPIC records and thorough charting.
  • Participates in Medical staff meetings and clinical teams.
  • Pursues continuing education to improve patient care quality.

License to practice as an APRN in Connecticut

Job purpose

To provide high quality health care to the patients of the Fair Haven Community Health Center in the primary care setting consistent with FHCHC policies and mission.

Duties and responsibilities
  • Provides primary medical care in accordance with the provider’s advanced practice specialty
  • Develops a plan of care for each patient, including medical history, physical examination, diagnosis and appropriate treatment and/or referral, including hospitalization if necessary
  • Uses all available resources in diagnosis and treatment, such as laboratory and others tests in an appropriate and cost efficient manner
  • Confers with other clinicians, patient’s families and other support staff concerning treatment and care of patients when appropriate
  • Refers those cases which require specialist services, but maintains primary care responsibility to assure continuity of care
  • Maintains accurate medical records within the EPIC electronic health record system and performs appropriate charting and follow up on patients
  • Participates in Medical & General staff meetings and appropriate clinical teams
  • Participates in the Health Center’s Performance Improvement program, in other task groups as needed, and pursues continuing education opportunities to update medical knowledge and assure quality patient care
Qualifications
  • Graduation from an accredited APRN program
  • License to practice as an APRN in State of Connecticut
  • Qualified in Basic Life Support techniques
  • Bi-lingual in Spanish and English highly desirable
