Job summary

Yale New Haven Health is seeking a physician to provide high-quality non-surgical primary and continuing care within Northeast Medical Group. The role includes leading an interdisciplinary team and coordinating care while upholding professional ethics and patient-centered values.

Qualifications

• MD or DO degree with ABIM/internal medicine residency (3 years minimum) or ABFM/family medicine residency (3 years minimum)
• Valid American Board certification in internal medicine or family medicine
• Connecticut medical license or eligibility
• BLS certification
• DEA registration and Controlled Substance Certificate

Responsibilities

• Orders, performs and interprets diagnostic tests
• Involves patients and families in care and treatment decisions

Responsibilities

Responsible for providing appropriate non-surgical primary and continuing care to all patients in Northeast Medical Group, as well as leadership, coordination, and administration duties. The Physician leads an interdisciplinary team of primary caregivers and administrative support staff to perform assigned duties while displaying the highest standards of ethical and moral conduct as well as acting in the best interest of Northeast Medical Group supporting our mission, vision and values to optimize the health and well-being of our patients.

• Evaluate patients, and treat acute and chronic illnesses
• Orders, performs and interprets diagnostic tests
• Involves patients and their families in care and treatment decisions
• Creates treatment plans and completes orders in patient's charts
• Collaborates on improvements in care delivery and service with staff, other physicians and management
• Initiates appropriate specialty referrals
• Provides clinical information to the interdisciplinary team in a clear and timely manner
• Ensures that proper patient documentation is timely, accurate and complete
• Works with practice management to ensure proper compliance with billing, coding and CPT code assignment
• Achieves and maintains optimal patient access
• Provides interdepartmental medical consultation as requested
• Participates in the development of medical protocols, standard procedures, and management of patients
