Job summary

Yale New Haven Health is seeking a board-certified or board-eligible physician in either internal medicine or family medicine to join a busy, collegial practice. The role emphasizes patient-centered care with EPIC EMR usage and a focus on work-life balance in a community near NYC/New Haven.

Responsibilities

• Evaluate patients and treat acute and chronic illnesses.
• Orders, performs and interprets diagnostic tests.
• Involves patients and families in decisions about care and treatment.
• Creates treatment plans and completes orders in patient charts.
• Collaborates on care delivery improvements with staff and physicians.
• Initiates appropriate specialty referrals when needed.
• Provides clinical information to the interdisciplinary team in a timely manner.
• Ensures proper patient documentation is timely and complete.
• Works with practice management for billing, coding and CPT assignment.
• Aims for optimal patient access and participates in protocol development.

• Degree of MD or DO with ABIM-recognized internal medicine residency (three years minimum) or ABFM-recognized family medicine residency (three years minimum)

Licensure

• Valid American Board certification in either internal medicine or family medicine and renewed (if needed) prior to expiration Valid and unrestricted Connecticut medical license BLS certification Valid DEA registration and Controlled Substance Certificate or in the process of obtaining.
