Job summary

Qualifications

Responsibilities

• Provide direct patient care including examinations, assessments, diagnoses, and treatment.
• Prescribe pharmaceuticals and manage treatment plans when appropriate.
• Review patient histories, maintain medical records, and prepare reports.
• Supervise physician assistants / nurse practitioners as needed.
• Collaborate with care coordination staff for multi-disciplinary care.
• Participate in health promotion and preventive programs.
• Cover after-hours calls as required.
• Oversee transitions of care for patients with multiple hospitalizations.
