As the county hospital system, JPS has been serving the residents of Tarrant County for over 100 years. Job Summary:

• Outpatient Clinic
• Call (Minimal)
• Patient panel – Family Medicine

Duties and Essential Job Functions:

• Coordinate multidisciplinary care to optimize patient outcomes.
• Review patient records, diagnose, treat, and provide care management and education.
• Make appropriate patient referrals.
• Monitor health guidelines established by JPS and Acclaim.
• Provide evidence-based, compassionate care to improve patient outcomes and satisfaction.
• Maintain collaboration with other healthcare professionals and staff.
• Support academic, educational, and research activities.
• Supervise Advanced Practice Professionals.
• Participate in Performance Improvement and Peer Review meetings.
• Partner and communicate effectively to enhance the patient and community experience.
• Serve as a resource to the community.
• Perform other related duties as assigned.

Knowledge, Skills, and Abilities:

• Comfort working in a large public/county facility.
• Knowledge of assessment, diagnosis, and treatment in the specialty area.
• Good interpersonal and communication skills.
• Self-directed with initiative in community activities and outreach.
