Neighborhood Health Association (NHA) is seeking a physician to join our PCMH Care Team in Northwest Ohio, providing high-quality medical care for outpatient and inpatient status. The role focuses on coordinating care with patients and team members to improve access, quality, and patient self-management.
The physician will work under the Medical Director, with responsibilities including documentation, CME, and meeting participation, while supporting underserved communities.
Monday – Friday schedule. No late nights, no weekends, and 11 paid holidays.
Responsible for providing quality medical care, specific to specialty, to NHA patients on both outpatient and in-patient status.
The Physician is a member of the (Patient Centered Medical Home) PCMH Care Team. All members of the PCMH Care Team will work cooperatively with and interact with the patients and other team members to coordinate patient care and achieve the stated patient and PCMH Team goals and objectives. PCMH goals and objectives include increased access to patient care, high quality patient care, and patient self-management of care.
