Job summary

U.S. Department of Veterans Affairs in Ohio is seeking a qualified primary care provider to deliver comprehensive care to a defined panel of Veterans.

The role leads a Patient Aligned Care Team (PACT) and collaborates with specialists to ensure high-quality care and continuity for patients served at the VA facility. The position emphasizes patient-centered care, interdisciplinary collaboration, and adherence to VA performance standards.

Responsibilities

• Provides comprehensive primary care services to a defined panel of patients
• History taking, physical examination, ordering laboratories and diagnostic studies
• Diagnosis and formulation of treatment plans for acute and chronic conditions
• Leads the Patient Aligned Care Team (PACT) including RN, LPN, and clerical staff
• Documentation of patient care in clinical records and coordination with specialists as needed
• Mon-Fri 8:00am-4:30pm schedule and availability for patient care needs
