Job summary

Foundation Medical Partners / Southern NH Health in Nashua, NH seeks a compassionate and experienced Primary Care Physician Assistant to join a not-for-profit, hospital-affiliated team. You will deliver high-quality care across diverse populations with strong APP support in a collegial setting.

This hybrid role features 40 hours weekly, a balance of direct patient care and coordination, and a comprehensive benefits package including health, dental, STD/LTD, malpractice protection, generous PTO

Qualifications

• Certified Physician Assistant.
• 2+ years of Primary Care experience.
• Strong clinical skills, including assessment, diagnosis, and treatment planning.
• Excellent communication, collaboration, and interpersonal skills.
• Proficiency with EPIC EMR preferred.
• Ability to work independently and as part of a team.

Responsibilities

• Provide 40 hours per week with 50% direct patient care and 50% coordinating and communicating patient care.
• Deliver comprehensive care to a diverse patient population in Nashua, NH.
• Collaborate with a collegial team and rely on moderate APP support.

Job Description:

We are seeking a compassionate and experienced Primary Care Physician Assistant to join our dynamic team at Foundation Medical Partners / Southern NH Health. In this role, you will play a vital role in providing comprehensive care to a diverse patient population in Nashua, NH.

• Collegial practice with solid APP support
• W2, employed position with Foundation Medical Partners
• Not-for-Profit, 501(c) Organization
• 40 Hours – 50% direct patient care work and 50% coordinating and communicating patient care
• Clinical Affiliate of Massachusetts General Hospital
• Hybrid Work Schedule – Potential remote options once fully trained
