Job summary

SolutionHealth is seeking a compassionate and experienced Primary Care Physician Assistant to join our Nashua, NH team. The role combines 40 hours per week with 50% direct patient care and 50% coordinating and communicating patient care within a multidisciplinary team, in partnership with Foundation Medical Partners / Southern NH Health.

Qualifications

• Certified Physician Assistant with 2+ years of primary care experience.
• Strong clinical skills in assessment, diagnosis and treatment planning.
• Excellent communication, collaboration and interpersonal skills.
• Proficient with EPIC EMR.

Responsibilities

• Provide direct patient care in a primary care setting.
• Coordinate and communicate patient care with the care team.
• Participate in a hybrid work model and potential remote options after training.

In this role, you will play a vital role in providing comprehensive care to a diverse patient population in Nashua, NH. Collegial practice with solid APP support W2, employed position with Foundation Medical Partners Not-for-Profit, 501(c) Organization 40 Hours – 50% direct patient care work and 50% coordinating and communicating patient care Clinical Affiliate of Massachusetts General Hospital Hybrid Work Schedule – Potential remote options once fully trained Convenient Southern NH/MA Border Location, minutes to the mountains or ocean

Qualifications

• Certified Physician Assistant 2+ years of Primary Care experience

Work Shift

Mon-Friday 8-5, (With potential for hybrid remote work)
