Job summary

Foundation Medical Partners / Southern NH Health in Nashua, NH seeks an experienced Nurse Practitioner Assistant (PA) to join a collegial, outage-free practice with strong APP support. The role is 40% direct patient care and 50% care coordination, with a hybrid schedule and potential remote options after training.

The ideal candidate has 2+ years of Primary Care experience, excellent clinical skills, and proficiency with EPIC EMR.

Qualifications

• 2+ years of Primary Care experience required.
• Strong clinical skills in assessment, diagnosis, and treatment planning.
• Excellent communication and teamwork abilities.

Responsibilities

• Provide direct patient care in a primary care setting.
• Coordinate and communicate patient care with the care team.
• Collaborate with affiliated hospitals and partners to ensure continuity of care.

Qualifications

• Certified Physician Assistant 2+ years of Primary Care experience
• Strong clinical skills, including assessment, diagnosis, and treatment planning.

• Ability to work independently and as part of a team.

Work Shift

Mon-Friday 8-5, (With potential for hybrid remote work)
