Qualifications

• Certification/degree from an accredited Physician Assistant program.
• Two years of PA experience in a primary care practice, or a relevant specialty practice preferred.
• Current licensure as a PA-C by the State of Vermont and national certifications recognized by the Vermont State Board of Medical Practice (or eligible).
• Healthcare Provider CPR certification.
• DEA license.

Responsabilités

• Conduct patient assessments and monitor clinical progress.
• Coordinate care across settings during transitions from acute care to rehab, skilled nursing, long-term care, or home-based care.
• Document treatment plans and outcomes; educate patients and families.
• Ensure compliance with regulatory, quality, and safety standards.
• Collaborate with interdisciplinary teams including physicians, nurses, therapists, case managers, and caregivers.
