Qualifications

• NP: Master's in Nursing with MA license, prescriptive authority from MA DPH-DCP and APRN authorization
• PA: Active MA PA license and DEA registration
• BLS/ACLS certification required for all candidates
• Influenza vaccination required as a condition of employment
• Proficiency with computer systems, including MS Office
• 1–3 years of related work experience

Responsabilités

• Assess, plan, implement, and evaluate care for a patient panel
• Evaluate health status, history, exams, tests, and risk to establish diagnoses
• Develop and implement individualized care plans including treatment and medications
• Deliver care with focus on patient satisfaction and cost‑effectiveness
• Monitor responses and revise care plans; provide education and self-care support
• Provide patient orientation and appropriate referrals
• Collaborate with physicians and care team as needed
• Identify learning needs and assist with educational programs
