Twin Cities Physicians is seeking a Physician Assistant (PA-C) to join our Mobile Rounding Provider team serving senior living communities in Minnesota. You’ll deliver on-site primary care, manage chronic conditions, and coordinate care plans within an ongoing care model designed for accessibility and continuity.

• Active Minnesota PA-C license in good standing.
• Experience in geriatrics, family medicine, internal medicine, hospital medicine, home health, or post-acute care preferred.
• Strong clinical judgment and communication skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide on-site rounding care in assisted living, memory care, and long-term care communities.
• Perform assessments, chronic disease management, preventative care, and follow-up visits.
