Job summary

Twin Cities Physicians provides on-site primary care, wound care, and chronic disease management across 300+ senior living communities in Minnesota. This role focuses on mobile wound care and rounding, delivering high-quality care to medically complex residents in community settings.

As a Wound Care & Rounding Provider, you will oversee advanced wound cases, work with a multidisciplinary team, and develop specialized expertise in senior care medicine within a growing model of care.

Qualifications

• Active Minnesota PA-C license in good standing.
• Experience in wound care, geriatrics, primary care, post-acute care, internal medicine, or related settings preferred.
• Strong clinical judgment and confidence managing medically complex patient populations.
• Valid driver's license and reliable transportation.
• Commitment to high-quality, outcomes-driven senior care.

Responsibilities

• Provide on-site rounding and advanced wound care services across senior living communities.
• Manage pressure injuries, diabetic ulcers, surgical wounds, and other complex skin conditions.
• Support chronic disease management and preventive care initiatives.
• Collaborate with physicians, nurses, therapists, caregivers, and facility leadership.
