Healthcare is changing.
More seniors are choosing assisted living, memory care, and community-based settings than ever before. As a result, the demand for providers who can bring high-level medical care directly into these environments continues to grow rapidly.
Twin Cities Physicians has built its model around this shift.
Our providers deliver on-site primary care, wound care, and chronic disease management across 300+ senior living communities throughout Minnesota. By bringing healthcare directly to residents, we improve access, strengthen continuity of care, and help prevent unnecessary hospitalizations.
This is not a traditional Physician Assistant position.
This is an opportunity to become part of a growing specialty area where experienced clinicians can make a significant impact while building highly marketable expertise.
As a Wound Care & Rounding Provider, you'll work with medically complex patients, manage advanced wound cases, and provide ongoing clinical oversight within senior living communities. You'll become a trusted healthcare resource for residents, families, nurses, and community leadership while helping shape outcomes for some of the most vulnerable patient populations in healthcare.
For providers who enjoy challenging cases, long-term patient follow-up, and the opportunity to develop specialized expertise, this role offers a unique career path.
We proudly encourage active duty, reserve, National Guard, and veterans to apply.
The future of healthcare is increasingly moving beyond the walls of hospitals and clinics. Providers who understand senior living medicine, wound care, chronic disease management, and community-based care will continue to be in high demand.
If you're looking for an opportunity to build specialized expertise while helping shape the future of senior healthcare, we'd love to connect.
