Job summary

Twin Cities Physicians is seeking a Physician Assistant (PA) to join our team in the Greater Twin Cities Metro Area. This role focuses on advanced wound care and geriatric medicine, delivering on-site rounding, primary care, and wound care within senior living communities.

You will manage complex cases, collaborate with multidisciplinary teams, and contribute to improving residents' quality of life across Minnesota.

Qualifications

• Minnesota PA-C license in good standing required.
• Experience in wound care, geriatrics, primary care, internal medicine, family medicine or post-acute care preferred.
• Strong clinical judgment and ability to manage medically complex patient populations.

Responsibilities

• Deliver on-site rounding, primary care, and advanced wound care services.
• Manage pressure injuries, diabetic ulcers, surgical wounds, skin integrity concerns, and medically complex cases.
• Lead chronic disease management and preventative care initiatives.
• Collaborate with physicians, nurses, therapists, caregivers, and facility leadership.
• Help reduce avoidable hospitalizations through proactive clinical intervention and follow-up care.
• Communicate effectively with families, caregivers, and interdisciplinary teams.
• Complete documentation, medication management, diagnostics, and lab coordination.
