Twin Cities Physicians in Minnesota is seeking a Physician Assistant (PA-C) focused on advanced wound care and geriatric medicine to deliver on-site rounding for senior living communities.

This full-time role combines primary care with wound care expertise, managing complex cases, collaborating with interdisciplinary teams, and supporting residents’ healing and quality of life across multiple facilities.

Qualifications

• Active Minnesota PA-C license in good standing.
• Experience in wound care, geriatrics, primary care, internal medicine, family medicine, or post-acute care preferred.
• Strong clinical judgment and confidence managing medically complex patient populations.
• Valid driver's license and reliable transportation.

Responsibilities

• Deliver on-site rounding, primary care, and advanced wound care services.
• Manage pressure injuries, diabetic ulcers, surgical wounds, skin integrity concerns, and medically complex cases.
• Lead chronic disease management and preventative care initiatives.
• Collaborate with physicians, nurses, therapists, caregivers, and facility leadership.
• Help reduce avoidable hospitalizations through proactive clinical intervention and follow-up care.
• Communicate effectively with families, caregivers, and interdisciplinary teams.
• Complete documentation, medication management, diagnostics, and lab coordination.
