Description

Northwestern Medicine has an exciting opportunity for either a Nurse Practitioner or Physician Assistant to work in our outpatient NMH Transitional Care Clinic.

The NMH Transitional Care Clinic’s mission is to increase access and improve health outcomes for at-risk patients with complex medical and psychosocial needs through an integrated, interdisciplinary team-based approach. We provide high-quality, empowering, and compassionate transitional care to meet the needs of our patients, providers, and community partners. Follow-up care is typically provided for about 6 months after an acute care crisis, aiming to improve health, facilitate navigation of the medical system, and support transition to long-term primary care physicians.

Schedule: M-F, days; no nights, weekends, or holidays.

On call required about 9 weeks/year. MDs and APPs rotate pager call coverage, which is handled remotely without in-person visits.

Essential Functions: Demonstrate clinical expertise through patient care activities including diagnosis, management of health problems, collaboration with physicians, patient education, mentorship, quality improvement, and research.

Responsibilities: Conduct health assessments, provide direct clinical care, prescribe medications and therapies, order and interpret tests, evaluate patient needs, educate patients and families, document care, refer to specialists, and participate in quality and educational initiatives.

Qualifications: Master's degree (nursing or Physician Assistant), valid Illinois license, certification, credentialing, BLS certification, and at least 1 year of APP experience.
