Job Description: Northwestern Medicine seeks a Nurse Practitioner or Physician Assistant for our outpatient NMH Transitional Care Clinic. The clinic's mission is to increase access and improve outcomes for at-risk patients with complex needs through an interdisciplinary team-based approach, providing high-quality, compassionate transitional care. The role involves follow-up care for patients post-acute crisis, facilitating their health understanding, navigation, and transition to primary care (typically for about 6 months).
Schedule: Monday-Friday, days; no nights, weekends, or holidays. On-call required (~9 weeks/year). On-call involves pager coverage without in-person visits, rotating among MDs and APPs.
Essential Functions include: Clinical expertise in diagnosing and managing health problems, developing educational programs, mentoring students, participating in quality improvement and research, and advocating for patients and families.
Responsibilities include: Conducting health assessments, providing direct care, prescribing medications, ordering and interpreting tests, patient education, documentation, referrals, and participating in research and quality initiatives.
Qualifications: Master's degree from an accredited program, current Illinois RN and APRN licenses or Physician Assistant license, certification by a national certifying body, credentialing through NMH MSO, BLS certification, and at least one year of APP experience.
