Job summary

Hartford HealthCare invites Nurse Practitioners and Physician Assistants to join our 100% outpatient primary care team in Litchfield, CT. You will build your own patient panel in a supportive, collaborative environment with strong leadership and professional development.

The position is full-time, weekdays only, with no on-call requirements and access to Epic and advanced clinical technology to enhance care coordination and outcomes.

Qualifications

• Unrestricted CT medical license (or eligible)
• APRN (Family, Adult/Geri) or PA training
• 2+ years’ experience in primary care or relevant outpatient setting

Responsibilities

• Provide outpatient primary care for adults 18+
• Collaborate with physicians and other providers in a team-based setting
• Manage patient panels and coordinate care within Epic-based workflows
