Qualifications

• Physician Assistant position in Greensburg area.
• Collaborates with physicians and interdisciplinary teams.
• Focus on geriatric, chronically ill members and independence at home.

Responsibilities

• Perform health histories and in-depth medical exams for new members to determine Plans of Care.
• Initiate and review medical orders and identify opportunities to improve care.
• Carry out procedures, document care and recommendations for team members.
• Order, interpret and follow up on lab tests; coordinate with internal and external providers.
• Educate staff, members, caregivers, and families to maximize home independence.
• Assess and adjust Plans of Care based on feedback and evaluations.
