Chestnut Health is seeking a Physician Assistant to join our physiatry team in a Skilled Nursing Facility setting. You will collaborate with therapists, nurses, and physicians to evaluate patients and implement comprehensive rehabilitation plans.

You’ll monitor progress, educate patients and families, and contribute to discharge planning in a patient-centered, collaborative environment that prioritizes work-life balance and high-quality care.

Formación

• Active license to practice in the state of employment.
• Current NCCPA certification.
• Current DEA license required upon start date.

Responsabilidades

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive rehabilitation plans.
• Conduct patient assessments, including histories, physical exams, and diagnostic evaluations, to set rehabilitation goals.
• Monitor progress and adjust treatment plans to optimize functional outcomes and independence.
• Participate in multidisciplinary meetings, case conferences, and educational activities to enhance clinical knowledge.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries in EMR systems.
• Educate patients and families about the rehabilitation process, expected outcomes, and post-discharge care.

Essential Duties & Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions
• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care

Qualifications

• Actively Licensed to practice in the state of which employed
• Current Controlled Substance License (required upon start date)
• Current DEA License (required upon start date)
• NCCPA Certificate
• Desire to work in Physiatry/Physical Medicine and Rehabilitation
• Orthopedic Experience (nice to have)
• Physiatry Experience (nice to have)
• Skilled Nursing Experience (nice to have)

License/Certification

• Certified Physician Assistant (Required)
