Job summary

Chestnut Health invites a Physician Assistant to join our physiatry team serving residents in skilled nursing facilities. You will collaborate with therapists and nurses to design and implement rehab plans, perform assessments, and help patients regain function and independence.

You will monitor progress, document care in EMR, and participate in multidisciplinary meetings while educating patients and families about the rehab process and post-discharge care.

Qualifications

Active licensure to practice in the state of employment (Illinois).
Current Controlled Substance License required at start date.
Current DEA license required at start date.
NCCPA certification.
Desire to work in Physiatry/PM&R.
Orthopedic experience and PM&R/SNF experience are pluses.

Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive rehab plans.
Conduct patient assessments to determine rehabilitation goals and interventions.
Monitor progress and adjust treatment plans to optimize functional outcomes and independence.
Participate in multidisciplinary meetings, case conferences, and educational activities.
Document initial evaluations, treatment plans, progress notes, and discharges in EMR.
Educate patients and families about the rehab process and post-discharge care.

Essential Duties & Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs
Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions
Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence
Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills
Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems
Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care
