Job summary

Chestnut Health seeks a Physician Assistant to join our Physiatry/PM&R team in a patient-centered, collaborative setting. You will work with therapists, nurses, and other clinicians to design and implement comprehensive rehabilitation plans aimed at restoring function and independence.

Qualifications

Active Physician Assistant licensure in the practicing state.
Current Controlled Substance License required at start date.
Current DEA license required at start date.
NCCPA certification.
Interest in Physiatry/Physical Medicine & Rehabilitation.
Orthopedic experience a plus.
Physiatry experience a plus.
Skilled Nursing experience a plus.

Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans.
Perform thorough patient assessments including histories, exams, and diagnostics to guide rehabilitation goals.
Monitor patient progress and adjust plans to optimize function and independence.
Participate in multidisciplinary team meetings, case conferences, and ongoing education.
Maintain accurate documentation of evaluations, plans, progress, and discharge summaries in EMR systems.
Educate patients and families about the rehabilitation process, outcomes, and post-discharge care.

Essential Duties & Responsibilities:

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
