Job summary

We seek motivated Nurse Practitioners or Physician Assistants to join our mobile care team, performing health assessments, managing wounds, and coordinating care across settings while ensuring compliance with Medicare and state guidelines.

Qualifications

• Active and unrestricted Nurse Practitioner or Physician Assistant license in Nebraska or Iowa.
• Experience in wound care, home-based care, or geriatric medicine preferred.
• Reliable transportation and valid driver’s license. The position involves day-trip travel to patient homes and partner facilities.
• Strong clinical assessment, communication, and documentation skills.
• Self-starter comfortable working independently with mobile technology.

Responsibilities

• Provide advanced wound care treatment for patients with complex or non-healing wounds, including assessment, debridement, and treatment plans.
• Deliver mobile wound care services directly in patients' homes or care facilities.
• Diagnose and manage chronic and acute conditions using a comprehensive, whole-patient approach.
• Accurately document all patient encounters, treatments, and progress in the EMR system in accordance with organizational standards.
• Actively participate in care planning and coordinate with internal and external support teams to optimize outcomes.
• Ensure strict compliance with Medicare, state regulations, and organizational protocols governing wound care services.
• Other duties as assigned.
