Job summary

QuantumCare Wound Care in the region seeks Nurse Practitioners or Physician Assistants to join our expanding mobile wound care team. You will perform in-home and in-clinic visits, assess patients, manage chronic and acute conditions, and deliver advanced wound care with autonomy.

You will document encounters in the EMR, collaborate with care teams, and ensure compliance with Medicare and state regulations. A flexible, supportive environment is offered.

Qualifications

• Active and unrestricted NP or PA license in Nebraska or Iowa.
• Experience in wound care, home-based care, or geriatric medicine preferred.
• Reliable transportation and valid driver’s license.
• Strong clinical assessment, communication, and documentation skills.
• Self-starter comfortable working independently with mobile technology.

Responsibilities

• Provide advanced wound care treatment for patients with complex or non-healing wounds, including thorough assessment, debridement, and individualized treatment plans.
• Deliver mobile wound care services directly in patients' homes or care facilities, ensuring comfort, accessibility, and continuity of care.
• Diagnose and manage chronic and acute conditions using a comprehensive, whole-patient approach.
• Document all patient encounters, treatments, and progress in the EMR system in accordance with organizational standards.
• Actively participate in care planning and coordinate with internal and external support teams to optimize outcomes.
• Ensure strict compliance with Medicare, state regulations, and organizational protocols governing wound care services.
• Other duties as assigned.

We are seeking motivated Nurse Practitioners (NPs) or Physician Assistants (PAs) to join our expanding mobile care team.
