Opportunity

Multnomah County Health Department is seeking a visionary and dynamic leader in Integrated Clinical Services. The role focuses on comprehensive primary medical care for medically underserved communities, addressing health equity, prevention, and chronic disease management.

Responsibilities

• Conduct comprehensive health assessments, order and interpret diagnostic tests, and develop treatment plans for acute and chronic conditions.
• Utilize panel‑based information technology tools to prioritize proactive care, including preventive health screenings and chronic disease management.
• Follow up on utilization issues such as emergency department use and hospital admissions to improve patient outcomes.
• Maintain training on new or updated systems to support patient health maintenance and enhance quality of care.
• Identify needs for specialty and rehabilitative services and make appropriate referrals, documenting medical necessity in the EHR.
• Assess and counsel patients on preventive care and schedule high‑risk referrals when necessary.
• Provide clinical consultation to nurse practitioners, physician assistants, nurses, and support staff, prioritizing and directing the care team.
• Coordinate care with behavioral health providers to deliver integrated services.
• Provide coverage for clinical colleagues during absences and collaborate on clinic, department, and county‑wide initiatives.
• Complete patient documentation, manage electronic inbox, and tag records for follow‑up and collaboration.
• Enter encounter and event codes for accurate billing of internal and external medical services.

Qualifications

• Graduation from an accredited medical school and one year residency in a primary‑care setting.
