The Per Diem Advanced Practice Clinician joins Integrated Clinical Services, delivering professional, comprehensive nursing care across the Portland Metro Area. Department: Health Department. Job Type: On-Call.

Key Responsibilities

• Diagnosing, managing, and treating acute and chronic medication conditions, including prenatal care; conducting medical history and physical examinations; ordering and interpreting diagnostic tests; devising treatment plans; prescribing and monitoring medications; counseling patients; and reassessing patients as appropriate.
• Utilizing panel‑based information technology tools to prioritize and direct proactive care for provider panel patients, including preventive health screening and chronic disease management.
• Providing follow‑ups on utilization issues such as ED visits and hospital admissions.
• Establishing the need for specialty and rehabilitative services and making appropriate referrals.
• Assessing and counseling patients regarding preventive care needs and delivering services or making appropriate referrals.
• Determining the need and scheduling for high‑risk referrals.
• Seeking clinical consultation from other providers as needed and providing consultation to nurses and support staff.
• Prioritizing and directing activities of the care team; coordinating with Behavioral Health providers to effectively integrate care.
• Providing coverage for clinical colleagues in their absence.
• Completing patient documentation and managing electronic in‑basket according to established policies and procedures.
• Participating in quality improvement and assurance projects as needed.

Minimum Qualifications

• Master's Degree in Nursing (NP or PA) or equivalent.
• Three (3) to six (6) years of clinical experience.
• Licensed Nurse Practitioner or Physician Assistant in Oregon.
• CPR certification.

Preferred Qualifications

• Additional years of experience or advanced specialty training.
