EPIC Health is urgently hiring a Family Medicine/Med-Peds physician to join our growing team in Cicero, IL. This full-time outpatient role offers a competitive base salary and a path to leadership opportunities within a collaborative, community-focused healthcare model.

You will provide comprehensive primary care, manage acute and chronic conditions, perform wellness visits, and coordinate with care teams while enjoying a sign-on bonus, potential performance bonuses, and comprehensive benefits.

Formación

• MD or DO degree from an accredited medical school.
• Board Certified or Board Eligible in Family Medicine or Internal Medicine-Pediatrics (Med-Peds).
• Active Illinois medical license or ability to obtain one before starting.
• DEA registration and applicable controlled substance authorization, or ability to obtain as required.

Responsabilidades

• Provide comprehensive outpatient primary care within Family Medicine or Med-Peds.
• Diagnose and manage acute and chronic medical conditions.
• Perform annual wellness visits, preventive screenings, and routine physical examinations.
• Develop individualized treatment plans focused on prevention and chronic disease management.
• Educate patients on lifestyle modifications and disease management.
• Build lasting relationships with patients and families.
• Collaborate with Advanced Practice Providers and specialty teams.
• Coordinate referrals, diagnostics, and follow-up care.
• Maintain accurate EHR documentation.
• Support value-based care initiatives.

• Build strong, lasting relationships with patients and families.
• Collaborate with Advanced Practice Providers, Care Managers, Behavioral Health professionals, and specialty teams.
• Coordinate referrals, diagnostic services, and follow-up care.
• Maintain accurate and timely electronic health record (EHR) documentation.
• Follow applicable clinical, regulatory, and quality standards while supporting value-based care initiatives.

RequiredQualifications
