Job summary

EPIC Health is urgently hiring a Family Medicine or Med-Peds physician to join our growing team in the Chicago area. This is an outpatient, weekday schedule with competitive compensation and a focus on building lasting patient relationships in a community-focused setting.

This role offers a base salary of $250,000–$400,000 per year, with bonuses up to $100,000 and a $10,000 sign-on bonus. Benefits include medical, dental, and vision insurance, PTO, CME support, and malpractice coverage.

Qualifications

• MD or DO degree from an accredited medical school.
• Board Certified or Board Eligible in Family Medicine or Internal Medicine-Pediatrics.
• Active Illinois medical license or ability to obtain one before starting.
• DEA registration and applicable controlled-substance authorization.

Responsibilities

• Provide comprehensive outpatient primary care within the scope of Family Medicine or Med-Peds.
• Diagnose and manage acute and chronic medical conditions.
• Perform annual wellness visits, preventive screenings, and routine physical examinations.
• Develop individualized treatment plans focused on prevention, chronic disease management, and long-term wellness.
• Educate patients on lifestyle modifications, preventive health, and disease management.
• Build strong, lasting relationships with patients and families.
• Collaborate with Advanced Practice Providers, Care Managers, Behavioral Health professionals, and specialty teams.
• Coordinate referrals, diagnostic services, and follow-up care.
• Maintain accurate and timely EHR documentation.
• Follow applicable clinical, regulatory, and quality standards while supporting value-based care initiatives.

Education

MD or DO degree from an accredited medical school

Illinois medical license

RequiredQualifications

PreferredQualifications

• Spanish fluency is preferred but not required.

• Familiarity with value-based care, population health, or chronic disease management.
