Job summary

Cooper Norcross Health is seeking an On-Site Supervisor Physical Therapist to provide outpatient rehabilitation services at an off-site practice. You will evaluate, treat, and educate patients to restore movement, reduce pain, and maximize independence following injury or surgery.

You will work with a multidisciplinary team to deliver evidence-based care that supports real-world functional recovery. This role requires DPT or equivalent, state license or eligibility, CPR and BLS certification,

Qualifications

• Doctor of Physical Therapy (DPT) or Master's degree in Physical Therapy.
• Current state Physical Therapy license or eligibility; CPR and BLS certification.

The On-Site Supervisor functions as a Physical Therapist providing outpatient rehabilitation services in an off-site practice setting. The role is responsible for evaluating, treating, and educating patients to restore movement, improve function, reduce pain, and maximize independence following injury, illness, surgery, or changes in physical health. Through evidence-based rehabilitation and patient-centered care, the Physical Therapist supports safe recovery, prevents complications, and enhances patients' overall quality of life. The On-Site Supervisor collaborates with physicians, advanced practice providers, and other healthcare professionals to develop and implement individualized treatment plans that promote functional recovery and long-term wellness. Through skilled therapeutic intervention, patient education, and coordination of care, this position helps patients achieve their highest level of mobility and independence while supporting Cooper's mission of delivering exceptional, patient-centered healthcare.

Education Requirements

• Doctor of Physical Therapy (DPT) or Master’s degree from an accredited Physical Therapy program.

License/Certification Requirements

• Current state Physical Therapy license (or eligibility for licensure)
• CPR certification and BLS
