Continuum Rehab Group is seeking a licensed Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities. This role focuses on delivering rehabilitative services under the supervision of a Physical Therapist, contributing to patient recovery and quality of life.

The position offers opportunities for mentorship, ongoing education, and a clinical excellence program designed to support specialty certification pursued at company expense.

Qualifications

• Licensed PTA with state licensure.
• Open to therapists with all experience levels.
• Familiarity with evidence-based practice and rehab protocols.

Responsibilities

• Utilize evidence-based intervention to support clinical goals.
• Provide skilled therapy services under supervision of a Physical Therapist.
• Assess patient progress and communicate adjustments to the supervising PT.
• Counsel patients and families on treatment plans and expectations.
• Contribute to discharge planning and documentation per regulatory requirements.

Experience/Qualifications:

Licensed Physical Therapy Assistant

This position is open to therapists at all experience levels.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.

• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.

• Provides skilled therapy services to patients who need rehabilitation services.

• Assesses and document patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.

• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.

• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
