Job summary

Continuum Rehab Group in White Settlement, TX seeks a Licensed Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care facilities. Qualifications

• Licensed Physical Therapy Assistant in good standing.
• Experience in SNF/Long Term Care preferred.
• Ability to work under supervision of a Physical Therapist.

Responsibilities

• Provide rehabilitative, skilled, medically necessary treatment under PT supervision.
• Deliver services to patients with physical functioning disorders.
• Assess and document patient progress; adjust treatments with PT.
• Offer counseling to patients and families about physical disorders.
• Contribute to discharge planning per regulations.

Experience/Qualifications:

Licensed Physical Therapy Assistant

This position is open to therapists at all experience levels.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
