Continuum Rehab Group is seeking a Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities in San Antonio, Texas. This role focuses on compassionate patient care and building effective rehab programs to enhance recovery and quality of life for residents.

We offer mentorship, training, and a clinical excellence program with licensure reimbursement after one year.

Qualifications

• Licensed Physical Therapy Assistant.
• This position is open to therapists at all experience levels.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders under the supervision of a Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and documents patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
