Job summary

Emblem Home Health & Hospice in the West Phoenix area seeks a Licensed Physical Therapy Assistant (PTA) to provide hands-on care under a Physical Therapist's supervision, with a flexible schedule to support your personal life.

As a PTA you will deliver treatments, guide exercises, monitor progress, educate patients on mobility aids, and maintain notes. AZ PTA licensure, CAPTE-approved program completion, CPR, and a valid driver’s license are required.

Qualifications

• Licensed Physical Therapy Assistant (PTA) in Arizona.
• Graduate of a CAPTE-approved PTA program (passed national exam).
• Current CPR certification.
• Valid driver’s license, insurance, and reliable transportation.

Responsibilities

• Deliver physical therapy treatments per PT and physician’s plan of care.
• Guide patients through therapeutic exercises and mobility training.
• Monitor and report patient progress and condition changes.
• Educate patients on mobility aids and prosthetic devices.
• Maintain accurate clinical notes and participate in team meetings.
