Take Your Therapist Home. As a PTA in this role, you will become an integral teammate in our innovative Take Your Therapist Home Program.

Join us at Rivetus Rehab! Take Your Therapist Home is a program focused on Home Health Care in the greater Southfield area with a steady referral source from multiple skilled nursing facilities.

What is Take Your Therapist Home? In a world of shorter SNF stays, Take Your Therapist Home (TYTH) is a program where clinicians work directly with patients in the SNF and home health, providing continuity of care to improve patient outcomes.

Rivetus has been serving the geriatric post-acute community since 1998. We are a specialized rehabilitation company operated and managed at the executive level by licensed clinicians, providing continuity of care across the health care continuum in Skilled Nursing, Home Health, and Outpatient settings.

Facility Location

Troy – 925 West South Boulevard Troy, MI 48085. This is a hybrid role providing Home Health therapy services in conjunction with SNF-based therapy services. Home Health geographic area is the vicinity around our Troy facility. Home care services are paid per visit, and SNF services are paid at an hourly rate. Ideal candidates would be within the vicinity of Troy.

Schedule

Monday - Friday, Full Time.

Required Qualifications

• Successful completion of an accredited Physical Therapy Assistant program.
• Licensure as required by the State of Michigan.

Skills and Abilities

• Ability to exercise professional judgment and initiative when analyzing problems and recommending solutions.
• Initiative and flexibility in the work environment.
• Ability to react effectively to the needs of patients and their families.
• Physical ability to bend, position, transfer and lift patients weighing from 80 pounds to over 300 pounds, with appropriate lifting devices as clinically recommended.
