Job description

As a Physical Therapy Assistant in this role, you will become an integral teammate in our innovative Take Your Therapist Home (TYTH) program. You will provide home health therapy services in conjunction with skilled nursing facility (SNF) based therapy services, supporting patient continuity of care across the health care continuum.

Scope

This hybrid role focuses on the greater Dublin area, with a steady referral source from multiple skilled nursing facilities. Home health geographic area surrounds our Shorepointe facility at 26001 E. Jeffereson St., Clarksburg, MI 48081. You will deliver services per visit (home health) and hourly (SNF).

Required Qualifications
