Job summary

Santa Clarita Post Acute is seeking a full-time Physical Therapy Assistant to join our in-house Rehabilitation Team in Santa Clarita, CA. You will evaluate and treat residents with physical, cognitive, and psychosocial impairments to improve independence and quality of life.

Responsibilities include implementing treatment plans under the supervision of a Physical Therapist, documenting progress, and communicating with staff and families.

Qualifications

• CPR certification is mandatory.
• Licensed Physical Therapy Assistant required.
• Preferably 1–2 years of long-term care experience.
• Must be in good health and able to meet physical demands.

Responsibilities

• Evaluate and treat residents with physical, cognitive, and psychosocial impairments.
• Administer treatment and rehabilitation services under supervision of PT.
• Document progress and outcomes of patient care.
• Follow treatment plans and communicate with staff and PT.
• Assist in creating and adjusting teaching plans; involve family when needed.
