Job summary

Santa Clarita Post Acute is seeking a full-time Physical Therapy Assistant to join our in-house rehab team. You will evaluate and treat residents to improve independence and quality of life under supervision of the Rehabilitation Director.

Responsibilities include administering treatments, documenting progress, and contributing to teaching plans while ensuring safety and adherence to clinical standards. Competitive wages and benefits offered.

Qualifications

• CPR certification required.
• Licensed Physical Therapy Assistant required.
• 1–2 years of long-term care experience preferred.
• Good physical and mental health required.
• Capable of lifting up to 40 pounds.
• Proficient in following oral and written instructions.
• Experience in supervising responsibilities is a plus.

Responsibilities

• Evaluate and treat residents with physical, cognitive, and psychosocial impairments.
• Administer treatment and rehabilitation services; conduct resident exercises.
• Follow treatment plans provided by supervising Physical Therapist.
• Evaluate treatment effects and document resident progress.
• Monitor and document resident responses to therapy.
• Contribute to teaching plans and involve family when appropriate.
