Continuum Rehab Group is seeking a Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care facilities in St. Charles, MO.

You'll work closely with licensed therapists to provide patient-centered rehabilitation. We offer a competitive total compensation package including a sign-on bonus, 401k, PTO, medical, dental and vision benefits, and licensure reimbursement after one year.

Qualifications

• Licensed Physical Therapy Assistant eligible to practice.
• Open to therapists at all experience levels.
• Experience in Skilled Nursing/Long Term Care facilities preferred.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides skilled and medically necessary treatment under supervision of a PT.
• Delivers rehabilitation services to patients needing therapy.
• Assesses progress and communicates with supervisor to adjust plans.
• Counsels patients and families about physical disorders and treatment goals.
• Contributes to discharge planning with the supervising PT.

Experience/Qualifications:

Licensed Physical Therapy Assistant

This position is open to therapists at all experience levels.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
