Job summary

Continuum Rehab Group in Raymore, MO is seeking a licensed Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care facilities.

You will provide evidence-based, skilled therapy under the supervision of a Physical Therapist, participate in mentorship programs, and help residents improve mobility and quality of life.

Qualifications

• Licensed Physical Therapy Assistant.
• Open to therapists at all experience levels.
• Under supervision and direction of a Physical Therapist.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of a Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and documents patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
