Job summary

Excelin Health in Fremont, CA is seeking a qualified Physical Therapy Assistant to deliver direct therapy under the supervision of a licensed Physical Therapist and in accordance with the physician's plan of care.

You will assist patients with therapeutic exercises, gait training, and use of assistive devices; apply modalities such as heat, cold, electrical stimulation, ultrasound and massage; and maintain clinical notes and patient records.

Qualifications

• Graduate of an accredited PTA program with an AA in science or applied science.
• Current CPR certification.
• Current state license or certification as required by state law.
• Must be a licensed driver with insured vehicle.

Responsibilities

• Provide direct physical therapy under supervision per plan of care.
• Assist patient in therapeutic exercises and gait training.
• Use modalities such as heat, cold, ultrasound, and massage.
• Instruct patients on wheelchairs, braces, crutches, canes, prosthetics/orthotics.
• Maintain and document clinical records.
• Participate in Home Care Team meetings.
