Job summary

Qualifications

Responsibilities

• Assist physical therapists in assessments and rehabilitation planning.
• Implement individualized treatment plans with exercises and modalities.
• Provide hands-on assistance ensuring safety and proper technique.
• Document resident progress and outcomes per policy.
• Coordinate with care team and families; participate in discharge planning.
