What you will do in this role:

• Provide compassionate, quality care in a team setting within a secure environment.
• Implement patients' Care Plans under the supervision of a Physical Therapist, assisting mentally and physically impaired patients in tasks to restore and enhance their performance.
• Communicate patient progress or issues to supervisors and team members; assist with patient scheduling and daily documentation of charges.
• Document patient care in compliance with regulatory, licensing, payer, and accreditation standards.
• Instruct patients' families or nursing staff on follow-up programs.
• Maintain equipment and work areas in a safe and clean condition.
• Adhere to the Company's Code of Business Conduct, the Corporate Compliance Agreement, professional standards, and applicable laws.
• Perform other duties as assigned.

Minimum qualifications:
